Berberine – a Natural Plant Compound – vs Metformin in Human Trials
Two groundbreaking studies show that berberine – a natural compound found in plants like barberry – can dramatically improve blood sugar, reverse prediabetes, and even outperform the world’s #1 diabetes drug on cholesterol and triglycerides.
The Science Behind Berberine
Study 1 – Prediabetes Reversal (Panigrahi et al., 2023)
This was a double-blind randomized controlled trial – the gold standard in science.
They took 34 adults with prediabetes – their HbA1c was between 5.7 and 6.4%, and their fasting glucose was between 100 and 125 mg/dL. That’s the very definition of prediabetes.
Half the group took a placebo. The other half took 500 milligrams of berberine three times a day – that’s 1,500 milligrams total. No other interventions. No diet change. No exercise program.
After 12 weeks, the results were incredible:
- Their HbA1c dropped from 6.4% to 5.43% – that’s a nearly 1% drop, which effectively reversed prediabetes back into the normal range.
- Their fasting glucose went from 122 down to 96 mg/dL – again, totally normal.
- Their 2-hour glucose on the OGTT dropped by about 37 mg/dL.
- Insulin resistance, measured by HOMA-IR, improved by 20 to 33%.
- And here’s something else – not a single person in the berberine group progressed to diabetes.
And there were no negative side effects, no issues with the liver or kidneys. All from this one supplement.
Study 2 – Berberine vs. Metformin (Yin et al., 2008)
Now if you’re already living with type 2 diabetes, here’s another fascinating study.
In a head-to-head randomized controlled trial, 36 newly diagnosed type 2 diabetics were given either 1,500 mg of berberine per day, or 1,500 mg of metformin per day.
After 13 weeks:
- HbA1c dropped by 1.9% in the berberine group, compared to 1.4% in the metformin group.
- Fasting blood glucose fell by about 68 mg/dL with berberine, compared to 50 mg/dL with metformin.
- Post-meal glucose improvements were similar between the two groups.
- But here’s the kicker – berberine actually lowered triglycerides and cholesterol more than metformin.
Think about that: a single natural compound performed just as well as one of the most commonly prescribed diabetes drugs… and even outperformed it on cholesterol and triglycerides.

The research shows that 1,500 milligrams per day – usually split into three doses of 500 mg with meals – can make a profound difference. We’re talking about bringing fasting glucose and HbA1c back into the normal range, and even matching the effects of metformin.
It’s rare that a single natural compound can make such a dramatic difference. And yet here we are.
